Entry
into A Touch of Care begins
with a phone call requesting service. Payment sources are verified
for each client and family. Our professional RN will schedule
an appointment for a complete in-home assessment of the client and
family needs. The Registered Nurse assessment visit allows gathering
of medical information regarding the client's functional abilities,
medications, nutritional status, and specific care requirements. From
this information, the RN will write the client's individualized Plan
of Care. Our
caregivers will follow this Plan of Care in assisting your loved one
with care at home.
In Home Aide Services: Care
and service will be delivered in accordance with the client's Plan
of Care, written by our RN. Caregiver duties may include,
but are not limited to the following:help with prescribed exercises
which the client and Aide have been taught by an appropriate professional;
- provide or assist with personal
care (i.e., bathing, mouth care, skin care, hair care);
- assist with ambulation;
- assist client with self-administration
of medications which are ordered by an MD or person authorized
by state law to prescribe;
- perform incidental household services which
are essential to the client's care at home;
- record and report changes in the client's
condition, family situation or needs to the appropriate health
care professional.
